Client Consent

I, the undersigned, do hereby request and consent to a home organizing project by Leave it to Alyssa (“Business”). I wish to rely on the Business to exercise judgment for my best interest, the below-named client, during the organizing process. I will inform the Business of any items I wish to keep and give permission for the Business to remove unwanted items, expired items, and damaged items, and agree those items will then be property of the Business.

I understand that organizing is not an exact science. I further understand and accept that fees are paid for performance of organizing only, and not a guaranteed result. I acknowledge by my signature below that although a good outcome is expected, and a reasonable effort has been made to establish realistic expectations, there cannot be any warranty, expressed or implied, as to the results that may be obtained.

I understand that any questions I may have regarding the organizing project may be directed to Alyssa Kragor before and during said project. I acknowledge I may request a copy of this document.

Client Consent for Photography

I, as the client identified above (“Client”), consent to have photographs, videotapes, digital or audio recordings, and/or images of the organizing project (collectively, “Photography”), taken by Leave it to Alyssa. I understand that such Photography will be recorded to document and assist with the business’ social media and to assist with business operations. 

I understand that the Photography or a portion of the Photography may become part of Leave it to Alyssa’s advertising. I further understand that the Business will own the Photography and I will not receive any payment for such Photography, but that I will be allowed to access or view the Photography that becomes part of advertising.  

I have read this consent in its entirety and agree to be bound by all its terms and conditions as described above. I acknowledge and agree that I have been given the opportunity to ask any questions and had all my questions answered to my satisfaction. 

Client Financial Responsibility Agreement

I clearly understand and agree that all services rendered to me may be charged directly to me, and that I am personally responsible for full payment immediately following the completion of the organizing project. I understand that even if I suspend or terminate services, any fees for professional services and products rendered to me will be immediately due and payable. 

I acknowledge that I am responsible for any outstanding fees for services provided to me by Leave it to Alyssa. Services fees are $85.00 per each hour of organizing services and per organizer, including off-site work. If project location is more than 20 miles outside of the Canton, GA 30114 area, there will be an hourly charge for travel time. Materials purchased by Company will be charged at the manufacturer’s suggested retail price plus state tax. Checks that cannot be processed by the bank will result in a $50 fee.

All projects require a $50.00 nonrefundable consultation fee. This payment will be due prior to the

Discovery session and can be paid via check, Venmo, or PayPal. If you move forward with the organizing project, the $50.00 will be deducted from your total invoice amount. Any other arrangements that may involve a payment plan or payment deferral must be made in writing by Alyssa Kragor. Verbal agreements are not acceptable.

I acknowledge that the Company reserves the right to charge a fee if I do not cancel the scheduled appointment without providing 24-hour prior notice to the Company. I further acknowledge that the Company reserves the right to reschedule my project appointment if I am more than 15 minutes late to the scheduled appointment.
